The ‘app’ needs to be added to each site. Because of the nature of the package, the installation cannot be deployed tenant-wide or globally.
Tenant-wide deployments work for some things, such as web parts, but not for other types, like Definitions.
Once your app is in the Tenant App Catalog, you can script the deployment for all sites. You must manually activate new sites or create a provisioning engine to apply it automatically.
PnP PowerShell to install apps is the most common way to do this, and you can feed it a .csv of all the sites, which can be exported from the admin center.
